What we collect through this website, why, and what we do with it.

This policy covers ablegivers.com. It explains what information this website collects, why we collect it, and what happens to it afterwards. It is written to be read, not to be impressive.
Referral forms. When somebody sends a referral, we collect what they type: the referral source’s name and contact details, and information about the person who needs care — name, date of birth, address, contact people, medical and care needs, and how care would be paid for. This is health information, and we treat it that way.
Job applications. When somebody applies to work with us, we collect their name, contact details, where they live, certifications, experience and availability. We deliberately do not ask for Social Security numbers, dates of birth or identification numbers through this website. Those are collected on paper once we meet you.
Nothing else. This site sets no advertising cookies, runs no tracking pixels, and does not sell or share anything with data brokers.
To respond to referrals and arrange care, to review job applications, and to contact the person who got in touch. That is all. We do not use this information for marketing to people who have not asked to hear from us.
Form submissions are emailed to our office and a copy is stored on our web server in a folder that is not reachable from the internet. Ordinary email is not an encrypted channel. That is why the referral form asks people to send only what is needed to start a referral, and why we collect the rest by phone or through our secure clinical system. If you would rather not send health information by web form at all, call us instead and we will take it over the phone.
We share information only where it is needed to arrange or pay for care: with the caregiver assigned to a case, with the payer or case manager handling the authorization, and with other providers involved in that person’s care, such as a hospice or home health agency. We share information when the law requires it. We do not sell information and we do not share it for advertising.
Health information about our clients is protected under HIPAA and Georgia law, and is handled under our Notice of Privacy Practices, which we provide when care begins. This website policy sits alongside that notice; where the two differ on client health information, the Notice of Privacy Practices governs.
Referrals that become clients are kept as part of the clinical record for as long as Georgia law requires. Referrals that do not proceed are kept only as long as needed to answer questions about them. Job applications are kept for the period required for employment records, and are used only for hiring.
Access to submissions is limited to staff who need it. Server copies sit outside the public website. Accounts are password protected. No system is perfect, and we will tell people promptly if something goes wrong that affects them.
You can ask what we hold about you, ask us to correct it, or ask us to delete information that we are not legally required to keep. Clients have additional rights under HIPAA, set out in our Notice of Privacy Practices. Email or call us and we will help.
This website is not directed at children and we do not knowingly collect information from them. Referrals concerning a minor are made by a parent, guardian or a professional acting on their behalf.
If this policy changes we will update the date at the top. Material changes will be noted on this page.
